SAN ANGELO, TX. (THECOUNT) — Cody Wade, 31, of San Angelo, Texas, has been arrested on an aggravated sexual assault charge after authorities said he sexually assaulted a nonverbal dementia patient at a nursing home facility. The San Angelo Police Department investigated the allegation involving a resident at AVIR, located on Knickerbocker Road.
San Angelo is located in Tom Green County in west-central Texas, approximately 200 miles west of Austin and part of the Concho Valley region. The city serves as a regional center for healthcare, education, and commerce in the area.

According to an affidavit, officers were dispatched to AVIR, a nursing home facility at 5455 Knickerbocker Road, after a reported sexual assault involving a resident. The affidavit states the victim has dementia, is nonverbal, and is considered a fall risk.
The investigation began after a woman received permission from the nursing home to install a camera inside the victim’s room to monitor the resident’s condition. According to investigators, the woman checked the camera remotely and observed Wade approaching the victim.
The affidavit states Wade sat the victim upright on the bed and allegedly engaged in a sexual act involving the resident. Investigators said the camera captured portions of the incident, although its placement limited the view of certain actions.
According to the affidavit, investigators reviewed the footage and observed Wade facing the victim while making forward movements with his shirt lifted and his hands positioned in front of his midsection outside the camera’s view. Investigators said Wade later moved away from the victim, adjusted his clothing, and placed the resident back on the bed.
The investigator later spoke with Wade, who was at the facility, according to court documents. Authorities said Wade acknowledged that he had done something wrong to a resident but initially did not describe the alleged conduct.
The affidavit states investigators provided Wade with a statement-of-fact form after asking him to write down what occurred. Authorities said Wade wrote, “I made a resident give me fellatio.”
Wade was arrested on July 21, 2026, on a charge of aggravated sexual assault. Court records indicate he was released on July 23, 2026, after posting a $500,000 bond.
The court documents associated with the case did not specify whether Wade was an employee of AVIR or otherwise affiliated with the nursing home facility.
The San Angelo Police Department continues to investigate the allegations. Court records do not indicate a final disposition of the case at this time.
