Four Children, Grandmother Die In Ohio Carbon Monoxide Tragedy

TOLEDO, OH. (THECOUNT)Liberty Coleman, Storm Coleman, Legacy “Ace” Coleman, Stone Coleman, and their grandmother, Candace McAllister, 36, died following a suspected carbon monoxide poisoning incident in Toledo, Ohio, according to authorities. Investigators believe the family was waiting inside a vehicle with a flat tire when deadly levels of carbon monoxide accumulated inside the car.

Toledo is located in northwestern Ohio near the Michigan border along Lake Erie. The incident occurred Wednesday in the 300 block of Phillips Avenue near Detroit Avenue.

Authorities said the family had reportedly stopped after experiencing a flat tire and remained inside the vehicle while awaiting assistance.

According to Toledo Fire Chief Allison Armstrong, the children’s father arrived to help and discovered the occupants unresponsive before calling 911.

Emergency responders rushed to the scene and initiated lifesaving efforts.

Officials said investigators later determined carbon monoxide poisoning is the suspected cause of the deaths.

According to information provided to family members, authorities found carbon monoxide levels inside the vehicle measured at approximately five times the lethal limit.

The tragedy deepened Friday when Legacy, known to family members as “Ace,” was removed from life support after suffering catastrophic injuries related to the incident.

Authorities have not released the exact ages of the children, but family members described them as young siblings whose lives centered around one another.

The children’s mother, Adriana Coleman, remembered Liberty as a natural leader and described Storm as a child who looked up to her older sister while maintaining her own personality.

Coleman said Legacy was affectionately known as her “wild child,” while Stone was the youngest member of the family.

She also remembered McAllister as someone who brought energy and joy wherever she went, describing her as “the life of the party.”

In interviews following the tragedy, Coleman spoke about the close bond she shared with her children and the difficulty of facing life without them.

Family members have asked for prayers and support as they navigate the devastating loss.

Authorities continue to investigate the circumstances surrounding the incident, including how carbon monoxide entered and accumulated inside the vehicle.

No additional information regarding the mechanical condition of the vehicle or other contributing factors has been released by investigators.

A fund has been established to assist the family with funeral expenses. According to reports, donations may be made through the Coleman-McAllister Fund at Fifth Third Bank locations.

The investigation remains ongoing.
