ELKHART, IN. (THECOUNT) — John Levy, of Elkhart, Indiana, has been identified as the victim in a fatal Monday night crash near South Bend.

Levy, 66, died following a head-on vehicle crash in Elkhart on Monday night around 6 p.m.

According to reports, Levy was operating a vehicle on Old U-S 33 near CR-3 in Elkhart, IN, when his vehicle was struck from behind by another vehicle, operated by 22-year-old Cody Nemec. The collision forced Levy’s vehicle into oncoming traffic where a head-on collision occurred.

Levy was transported to an area hospital where he was pronounced dead of blunt force trauma.

Nemec was taken to the hospital with cuts to his head, reports WSBT.

The people in the other vehicle were not injured.

No further information on the crash was immediately provided.

The cause of the crash remains under investigation.
